HOOP Privacy Policy

The privacy of your personal information is paramount. This privacy policy describes how we protect and handle the personal information that you provide and that we obtain through our web-based service www.gethoop.com and related web sites (“HOOP”).

Introduction

HOOP is offering a web-based platform to bring together you and healthcare providers to empower you to make the best and cost-efficient healthcare decisions.  HOOP offers cost transparency as well as quality ratings when available for our members who are searching for providers and these tools enable HOOP members to make better choices.    HOOP provides information on participating providers, organizes key information to help patients make informed choices in their search to independently assess particular providers, long-term care facilities, and other providers of healthcare services (collectively, “Healthcare Providers”).

Collection of Your Personal Information

You choose whether to create an account with HOOP. The first time you sign in to HOOP, you will be asked to create an account. To do so, you must provide personal information, such as your email and address.  This information enables the site to search and organize search results by your location.   For Paid members we will also collect relevant payment information for processing of online payments.  This information will be stored at a secured server of payment processing site, which uses industry standard encryption and security features to keep this information secure.    HOOP  does not store your payment information.    Your email address will be your username and you will be asked to create a password.

You can review and update your account information and modify, add, or delete any optional information by signing into your HOOP account and editing your account profile.

Account Access and Controls

By default, you are the custodian of any records you create. You can close your account at any time by signing into your HOOP account and editing your account profile.

How We Use Your Personal Information

We use personal information collected through HOOP to enable the services provided through HOOP and as described in this privacy statement. We do not use or disclose your information except as described in this privacy statement. In support of these uses, we may use personal information to:

– provide you with important information about HOOP, including critical updates and notifications;

– send you email and/or mobile alerts;

– notify of you of new participating providers in your area;

– suggest appropriate services and/or products relevant to you, your lifestyle and your geographic location

We may access and/or disclose your personal information if we believe such action is necessary to: (a) comply with the law or legal process served on us; (b) protect and defend our rights or property (including the enforcement of our agreements); or (c) act in urgent circumstances to protect the personal safety and welfare of HOOP users or members of the public. Personal information collected on HOOP is stored and processed in the United States.

We will not sell your personal identifiable data.

How We Use De-identified and Aggregated Information and Statistics

We may use de-identified and or aggregated information gathered from HOOP and related statistics to improve the services provided through HOOP, and for certain research and analysis including clinical medical research. We may also provide this information to our partners for such purposes. De-identified information does not include personally identifiable information such as name, email address, or address, but may include gender, height, weight, postal code, and medical information. Aggregated information is information from multiple users that is not associated with any individual user account.

Email and Mobile Alerts

To keep you informed of the latest improvements and alerts relating to out site, services or new participating providers HOOP may send you emails and or mobile alerts. You have the ability to opt out of receiving emails and/or mobile alerts.

Security of Your Personal Information

We are committed to protecting the security of your personal information. We use a variety of security technologies and procedures to help protect your personal information from unauthorized access, use, and disclosure. For example, we store the personal information you provide encrypted on computer servers with limited access that are located in controlled facilities. We restrict access to personal information to HOOP employees, contractors, and agents who need to know that information in order to operate, develop, or improve our services. These individuals are bound by confidentiality obligations and may be subject to discipline, including termination and criminal prosecution, if they fail to meet these obligations.

HOOP is not a “covered entity” under the Health Insurance Portability and Accountability Act of 1996 and the regulations promulgated thereunder (“HIPAA”). Although HOOP is not covered by HIPAA, we are committed to user privacy and have in place strict data security policies and measures, and ensure that users control access to their information. We let users know what information we collect when they use getHOOP.com, how we use it, and how we keep it safe.

Onward Transfer

We reserve the right to transfer some or all of Your Personal Information if HOOP becomes involved in a merger, acquisition, or any form of sale of all of its assets. Doing so will enable us to continue to provide service to you without significant interruption. We will provide notice before Personal Information is transferred and becomes subject to a different privacy policy.

Accessing and Updating Personal Information

You may change the settings of your HOOP account via the “settings” section of such account. You may organize or delete certain information through such settings or, alternately, terminate your HOOP account. Such deletions or terminations will take immediate effect. Residual copies of the deleted information and/or terminated HOOP account may take up to 60 days to be deleted from active servers and may remain in perpetuity in backup systems.
